Physicians Care Surgical Hospital cannot assist with records requests pertaining to your Rothman Orthopedic office visit.  Please contact your surgeon's office directly by clicking here.

To obtain records for your surgical procedure performed at Physicians Care Surgical Hospital, please print and complete the Authorization for Disclosure of Protected Health Information form (click here to download PDF).  The completed form can be sent to our Medical Records office as follows:

Once your request is received a member of our Medical Records team will contact you if there are any questions or need for follow-up.  Fees may apply per Pennsylvania Department of Health published guidelines as published here.

All fully completed requests will be fulfilled within 30 days of receipt of payment.
