Looking for records related to your visit at your surgeon's office? Please contact your surgeon's office.

To obtain records for your surgical procedure performed at Physicians Care Surgical Hospital, please print and complete the Authorization for Disclosure of Protected Health Information form (click to download PDF).  The completed form can be sent to our Medical Records office as follows:

3. By mail at 454 Enterprise Drive, Royersford, PA 19468

Once your request is received a member of our Medical Records team will contact you if there are any questions or need for follow-up.  Fees may apply per Pennsylvania Department of Health published guidelines as published here.

All fully completed requests will be fulfilled within 30 days of receipt of payment.
